HISTORY

In 2012, community leaders began discussing the idea of healthy community initiative for the Forest Lake area. Simultaneously Fairview Lakes Medical Center was in the midst of conducting their Community Health Needs Assessment which guides the work of the Community Health department over a multi-year period. The results of Fairview’s needs assessment, coupled with local public health department, state and national findings confirmed the need for communities to address the rise in obesity and related chronic diseases, including diabetes, cancer and heart disease.

In February of 2013, Fairview Lakes Medical Center, the City of Forest Lake, and Washington County Public Health and Environment convened a group of key stakeholders to assess interest in a health improvement initiative, and Health UP was born.
